The point-slope form is 
y-y1=m(x-x1)
where (x1,y1) is the point through which the line with slope m passes.

Therefore, with m=-4, point P(-1,5)
the equation is
(y-5)=-4(x-(-1))
=>
(y-5)=-4(x+1)
